Rabies pre-exposure vaccination is
indicated for persons traveling to areas of risk that
may not have the appropriate post-exposure treatment
resources available.
Pre-exposure vaccination can be one of
three types.
Human diploid cell rabies vaccine (HDCV)
Purified chick embryo cell (PCEC) vaccine
Rabies vaccine adsorbed (RVA)
CDC
Recommendations
When to give:
Serological testing for those in
continuous exposure to high risk environments ever 6
months. If antibody titer falls below acceptable levels
booster is required.
Contraindications
to Pre-exposure Vaccination:
Immunocompromised
patients should avoid vaccination. If exposure to risk
is unavoidable, and vaccination is necessary, titers
should be checked after vaccination.
Post Exposure Rabies Treatment
Treatment with RIG (Human Rabies
Immunoglobulin).
Current
CDC recommendations
When to give:
When not previously vaccinated, give as soon as possible
after exposure. RIG is given with pre-exposure
vaccine.
Contraindications:
Pregnancy is not a
contraindication to postexposure prophylaxis.
